Your Input is Important - Please take our Quick Survey!

 

The NH Council on Developmental Disabilities greatly values the input of people with disabilities, families and everyone the Council works with. The Council is required to submit the results of annual satisfaction surveys to the federal Administration on Developmental Disabilities (ADD), which provides funding for the Council and the activities that it supports.  Your feedback will help ensure that federal policymakers understand the impact of Council-supported activities on the lives of people with disabilities and their families.  The Council is also eager to receive your feedback to improve the quality and effectiveness of its future activities.     

Please respond even if you are only familiar with one Council-supported activity (such as a program or activity supported by a small grant from the Council) or publication (such as the RAP Sheet or Stepping Stones) or if your contact is limited to one staff person.  We hope to hear from those who know us through informal networking and sharing of information as well as more formal working relationships.  Surveys are short and easy to complete! 

There are two versions of our survey, one is for people with disabilities, families and those who provide direct hands-on support. The other is for stakeholders, which includes government agencies, provider organizations, non-profits and advocacy groups. If you fall into both categories, we encourage you to complete the survey for people, families and direct care providers.
